This picture is based on the fanfiction "falling" from Fantasia Wandering on ff.net.
Raphael is carrying April here over the wall of the Kraang facility while they bail her out in the first chapter. Donatello catches her on the other side of the wall.
I really love and adore this story and recommend it to everyone, who doesn't know it yet. Fantasia gives April more depth and fills the holes the series keeps open.
(this is after they freed April from the Kraang and defeated Snakeweed in the second episode.)
